Preparing for your first visit
Welcome! We look forward to seeing you soon. We want to make sure that your first visit goes smoothly, so here is a list of items to bring with you on your first visit:
- New Patient Forms – There are three pages in total. You can download these forms and fill them out at home, or come in 15 minutes prior to your appointment time, and fill them out in our office. Please take a look at, or download our Notice of Privacy Practices (HIPAA) before you complete page 2 of the new patient forms.
- Recent x-rays – Any dental x-rays taken within the past 12 months, or a complete series taken in the past 3 years, would be helpful to have during your initial exam. Digital x-rays can be forwarded to our main email address: firstname.lastname@example.org. If film x-rays were taken, please allow your prior dentist a minimum of 1 week to mail your x-rays to our office. Some dental offices require a release to be signed before releasing a copy of your x-rays. We can help! Just download and fill out the form entitled Request for Release of Patient Records, and send or fax this form to your other dentist.
- List of Medications – The top portion of our medical history form (the last page of the New Patient Forms) has a place for you to list your current medications. If your list is long, and you have already gone to the trouble of typing it up, skip that section and bring a copy that we can add to your chart.
- Dental Insurance Card – Our patient history form has a section that covers dental insurance. Just the same, if you have a dental insurance card, bring it with you. Sometimes it includes other information that we may find helpful. Medical coverage and dental coverage are usually handled separately, and by different insurance companies. Make sure the card you are looking at has “dental” listed somewhere on the front or back.
